• custom cut form, all notched plates and custom cut plates require this
• tax exempt number, Massachusetts businesses and Universities only

Credit Card Order(Visa, Mastercard, or American Express only):

• fax or phone credit card information (do not send by e-mail)
• shipping information
• catalog number, plate dimensions(include thickness), or system
• glass required: soda-lime, borofloat, or low-iron
• number of plates; notched or rectangular
• contact information, including e-mail address and phone number
• custom fabrication form, all notched plates and custom cut plates require this

International Orders :

• fax, phone, or e-mail purchase order information (include billing &shipping)
